COBRA
As you leave Sunland Asphalt, there are a few things you need to know about your optional COBRA benefits.
Your Medical, Dental, Vision, and Health Care Flexible Spending Account (HCFSA) coverage ends on the last day of the month in which your employment ends. You can continue coverage under COBRA. EBC, our COBRA administrator, will mail information regarding your COBRA continuation coverage rights to your home address, generally within 14 days after notification of your qualifying event.
How COBRA Works
Get your COBRA notice.
You and your dependents will receive a notice within 14 days of your last day of employment.
Decide about your coverage.
You have 60 days from the notice date to enroll in COBRA and continue your medical, dental, vision, and/or Health Care FSA coverage.
Pay your premium.
Your COBRA notice will list your premiums and explain how to make payments for your coverage.
