Low DHMO Plan
Lower paycheck cost and a copay schedule
- Preventive visits are covered 100% twice a year.
- All basic, major, and orthodontia services are covered after you pay the assigned copay.
- This plan is only available to those in certain ZIP codes. Log in to the Benefits Portal to see if this plan is offered to you.
High DPPO Plan
Higher paycheck cost and broader provider options
- Preventive visits are covered 100% twice a year.
- 80% coverage for basic services and 50% coverage for major services after deductible is met
- Orthodontia services available for children under age 19
- Both in-network and out-of-network coverage available
How It Works

Select dental coverage when you enroll.
You can enroll your spouse/partner and dependents too. Benefits begin on the first of the month following 30 days of full-time employment.
